Pursuant to the North Carolina Public Records Law, I hereby request the following records:

Records of any and all wage complaints filed against the following home health and hospice agencies for the inclusive period between January 1, 2010 and September 1, 2020. By "records" I mean any information including the date the complaint was filed, the nature of the complaint, the amount owed to the complainant, the dates worked but not compensated, and the outcome of the complaint.

LHC Group is a nationwide company that operates the other home care agencies as subsidiaries in the state of Pennsylvania. If necessary I can provide the name and addresses of all branch offices in the state.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Dear Mr. Bittle:

We are in receipt of your request, we have found a few files that may match your inquiry, however, we will like to discuss your options. If you can please call me and clarify.

1. Full Request: The administrative process for approval for a full redaction of all files and documents therein could take up to 90 days. Per department policy, there is a charge of $0.10 per page associated with your request for files larger than 50 pages. You will receive another notification with the amount of pages that your file contains, the cost for the file copy and payment instructions. Once payment is received for the amount noted, our office will forward you a copy of the file. The copies will be sent via US Postal Service, email, or fax.

1. Partial Request: The adminsitrative process for approval for a partial redaction could take up to 30 days. This option will only include the investigative report (case summary), citation and/or penalties assessed, no other documentation will be included. There will be NO cost associated with it.

Please let me know what option you choose so that I may proceed accordingly.
