I'm writing to obtain a copy of the critical incident report from New York City regarding a suicide in on December 14 in Queens.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Hi Gwynne - We are still working on processing your request. As the acknowledgment letter sent on Dec. 27 stated (see attached), a determination on whether the request is granted or denied will be made in approximately 20 business days, or we will notify you if additional time is needed.
As you can imagine, we receive numerous requests and address them as quickly as we can. We should have something more definitive for you in a couple of weeks. In the meantime, let me know if you have any questions.
