Pursuant to the New York Freedom of Information Law, I hereby request the following records:
A copy of a Burial-Transit Permit for a deceased person, as named below:
Anthony Salvatore Casso; date of birth is May 21, 1942; date of death is December 15, 2020.The deceased died a prisoner at United States Penitentiary, Tucson (Pima County), Arizona; attached please find a proof of death.
Please note: the request is not for a death certificate, but for a Burial-Transit Permit which is a separate document required under the Public Health Law ยง 4145, and exists as a standardized form, DOH-1555. The request is made in the assumption that the body of Casso was transported from Arizona to New York for disposition.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
