RE: Division of Cemeteries
Pursuant to the New York State Freedom of Information Law (Public Officers Law §87 et seq.), I am requesting access to or a copy of the aggregated data and/or database records from the annual financial reports submitted by cemetery corporations.
In particular, I am interested in the following parts of the reports:
1. Parts 1-3 of the Annual Financial Report of Cemetery Corporation (all cemeteries must complete Parts 1-3).
2. Part 4 of the Annual Financial Report of Cemetery Corporation, including Perpetual Care Reconciliation and PM Loans.
3 .Part 5 of the Annual Financial Report of Cemetery Corporation, including Pre-Need and Installment Sales.
4. Schedule A (2136-f).
5. Schedule B (2133-f).
6. Schedule C (2079).
If there are any fees involved in the process of retrieving these records, please inform me in advance. If some of this information is exempt from release, please release as much as possible under the law.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
