–A list of hospitals that have been found in violation of the law requiring sexual assault victims to receive emergency contraception upon request

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Unfortunately, the Division of Long Term Care Health Facility Survey and Field Operations only houses information for long term care facilities. The records you are seeking for "A list of hospitals that have been found in violation of the law requiring sexual assault victims to receive emergency contraception upon request" is not a record that the Division of Long Term Care would maintain. I am not certain where you would be able to obtain such a list, but you can try to see if the Division of Acute Care Certificate of Need and Licensing can better direct you as they deal with acute care facilities such as hospitals. I am closing your OPRA request W131567 as this is the incorrect division and you will need to do the following:
