Pursuant to the New York Freedom of Information Law and the obligations of county health departments under the New York State Local Government Records Retention and Disposition Schedule (LGS-1) for birth and death records, I hereby request the following records:
Live Birth Data including constant birth rate, long term, short term, and average birth rates for within the county or school district subdivisions for the current (2022/23) and previous six years by year.
Birth to Kindergarten Survival Ratio for the county or school district subdivisions used for the past ten years by year.
All communications to or from school districts, NYS Education Department, NYS Bureau of Vital Statistics, or NYS Department of Health containing these statistical tabulations and data, and any communications regarding requests for this data, responses from the county health department to these parties regarding live birth data and/or any abnormal vital statistics observations since November 2019.
I request that the records be provided in electronic format, either by email attachment or on a CD-ROM.

I apologize for any confusion regarding my previous response. In an effort to clarify my Freedom of Information Law request dated March 13, 2023, I am providing more specific details regarding the records I seek:
Live Birth Data: Please provide the following data for each school district subdivision within Nassau County for the current (2022/23) and the previous six years (2016/17, 2017/18, 2018/19, 2019/20, 2020/21, and 2021/22):
a. Constant birth rate
b. Long term birth rate
c. Short term birth rate
d. Average birth rate
Birth to Kindergarten Survival Ratio: Please provide the Birth to Kindergarten Survival Ratio for each school district subdivision within Nassau County for the past ten years (2013-2022) on a year-by-year basis.
Communications: Please provide all correspondence and documentation of communications, including but not limited to letters, emails, and memos, involving the following parties since November 2019:
a. School districts within Nassau County
b. NYS Education Department
c. NYS Bureau of Vital Statistics
d. NYS Department of Health
The communications I am interested in pertain specifically to the live birth data, statistical tabulations, and any abnormal vital statistics observations mentioned in points 1 and 2 above.
