To Whom It May Concern:

Pursuant to the Louisiana Public Records Law, I hereby request the following records:

A copy of the OMV's decisions on all personalized plate applications, both approved and denied, received from January 1, 2019, to the date this request is processed. This includes, at a minimum, the requested plate numbers/text, the final decision and any notes or comments on the office's reasons for approving or denying the applications.

I am NOT looking for any personal identifiable information such as first and last name, street number, or street name. La. R.S. 44:33(A)(1) requires that you redact or segregate non-releasable information, if you need to. If you deny any or all of this request, please cite each specific exemption you feel justifies the refusal to release the information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
