Call for service log re: Arizona State Prison - Kingman

To Whom It May Concern:

Pursuant to the state open records law, Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122 and 39-128, I hereby request the following records:

All logs of all calls for service related to Arizona State Prison - Kingman and all related incident reports. Please provide materials for as far back as they are kept. If materials are kept in multiple formats, provision of responsive documents on a rolling basis, beginning with the most recent and electronic records, would be greatly appreciated. Please include all available segregable data, including the date of the incident/call, the nature of the call, the number of officers/personnel deployed to the location, any available descriptions, etc. If additional responsive materials may be held by other neighboring departments, identification of those entities would be greatly appreciated.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
