Pursuant to the Michigan Freedom of Information Act, I hereby request the following records:

* Annual salary records for Michigan HHS employee Jose F. Aliaga (DOB 4/1977) from January 1, 2012 through December 31, 2017, and monthly salary records from January 1, 2018 through May 30, 2018. Mr. Aliaga is identified as an "Assistance Payment Worker-E" in online public records, but may hold or have held other positions. (

* Copies of any and applications, approvals or other documentation related to leaves of absence requested by HHS employee Jose F. Aliaga in calendar year 2018 to date and in calendar year 2014 in full, as required under Michigan Code 15.402.

*Personnel records, including any disciplinary actions, for Jose F. Aliaga as an employee of HHS (to the extent allowed under Bradley, supra; Herald Company Inc. v. Ann Arbor Public Schools, 224 Mich. App. 266, 568 N.W. 2d 411, 1997.)

If fulfillment of any part of this request will result in a delay of other available above requested material, I ask that the records be released as they become available.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
