Pursuant to the Massachusetts Public Records Law, I hereby request the following records:

A copy of the investigation report of management of the OCME completed in or around August 2007 by the public safety consulting firm Vance.

This report was quoted in several media outlets — including, for example, the Berkshire Eagle [see: http://www.berkshireeagle.com/stories/patrick-fires-medical-examiner,460193] — as follows:

"The Office of the Chief Medical Examiner is in need of a complete overhaul," detailed the report released yesterday by Vance, a public safety consulting firm.

The report also found that the office has no written procedures on how to transport bodies, an overstocked caseload and little protection when it comes to the health and safety of employees. The new Medical Investigative Program, which was created by Flomenbaum, alienated public safety officials, the report found.

The report is also noted in the attached 2017 report from the Office of the State Auditor: "Despite the absence of a written ICP and Risk Assessment Plan, since 2003 the OCME has undergone no fewer than six independent reviews and assessments from outside agencies to include the Department of Public Health (2007), the Division of Occupational Safety (2007), Vance Report (2007)..." (See p. 25).

I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records.

I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
