Standard Contractor Evaluation Forms for McCourt Construction

To Whom It May Concern:

Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:

Any and all Standard Contractor Evaluation Forms filed since Jan. 1, 2009 for all public and private projects completed by McCourt Construction Company, of 60 K St., Boston MA 02127.

According to instructions for the form posted on DCAMM's website, the forms should be submitted to the following address:

"Commonwealth of Massachusetts, DCAMM – 15th Floor, ATTN.: Contractor Certification Office, One Ashburton Place, Boston, MA 02108; or by fax to (617) 727-8284; or by email to certeval.dcamm@state.ma.us"

I also request that all fees be waived as I am a journalist -- employed by MuckRock and the Boston Globe -- and intend to use any pertinent information I receive as a result of this request to educate the public.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
