Pursuant to the Massachusetts Public Records Law, M.G.L. c.66, §10, I hereby request the following records:

-- Copies of the most recent annual reports filed with the Attorney General's office by approved insurance carriers detailing their policies, procedures, and plan benefits as they relate to parity in the coverage of mental health and substance use disorders.

-- Copies of any notices of violation, reprimand, or comparable documents since 2014 sent to insurance carriers regarding their non-compliance with mental health/substance use disorder parity laws and regulations.

I also ask that all fees be waived for this request as I am a journalist employed by the Lowell Sun and I intend to use the requested records to publish articles in the public interest and not for any commercial purposes.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

Thanks for reaching out on this. I always feel (a little) bad asking people to go hunting through hundreds of pages of documents.

As we discussed, I'l like to amend my request to exclude the certification documents filed with the reports and limit the search to reports submitted by Blue Cross Blue Shield and Harvard Pilgrim Health Care.
