Lewisville, TX Police Department - Police Reports & 911 Calls on Requestor

To Whom It May Concern:

Pursuant to the Texas Public Information Act, I hereby request the following records:

I would like all 911 and police non-emergency call transcripts (or audio if no transcripts are available) and police reports since January 1, 2018 that involve:

1.) my name (whether it is written as Thomas Dustin Collinsworth or Thomas Collinsworth, and include all records with any and any combination of misspellings, abbreviations, initials, nicknames, or any shortened version of my name); or
2.) my address (662 Marcus Drive, Lewisville, TX 75057).

Besides my name and address, please redact all private information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request.

I would prefer the request filled electronically, by e-mail attachment. For files that are too large to be sent by email, please use an online file sharing service such as Dropbox, Google Drive, or your own servers and provide a link. Or you can upload the files directly using the link provided by MuckRock.
