From: Alice Carabajal

To Whom It May Concern:

Pursuant to the New Mexico Inspection of Public Records Act, I hereby request the following records:

I think I have evidence of fraud performed by Nezzy Care of Las Cruces, LLC on a iPhone that was provided to me by the company. I have notified the OIG, the OIG weeks ago, and the FBI, today. Yesterday, I was called by one of your officers, a Sargent Garcia badge number 976 at 12:18 stating that Nezzy Care is going to file embezzlement charges against me for the equipment. I am more than willing to return the equipment, but would respectfully request that it be preserved legally and be investigated by your fraud department as I stated to Sgt. Garcia. Sgt Garcia just reiterated that the equipment needs to be returned. I am requesting the call recording which occurred between me and Sgt Garcia yesterday 10/14/2022 at 12:18. The victims of this fraud are protected under the DDW Program financed by the federal government and state funds. These individuals are either mentally ill or intellectually disabled, they cannot speak for themselves and it is incumbent on the state oversight RN to advocate for them. I believe the cell phone has more evidence of civil law violations i.e. discrimination, bullying, and harassment. I sincerely hope Sgt. Garcia was only doing his job making the call, but the fact he did not listen to my concerns and forward to you (or maybe he did) but left the weekend to consider that I may be violating the law, I was compelled to pen this letter.

Thank you for your time and consideration,
Alice Carabajal, RN

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 15 business days, as the statute requires.

Sincerely,

Alice Carabajal
