To Whom It May Concern:
Pursuant to the the Louisiana Public Records Act, I hereby request the following records:
Copies of all receipts written since 2010 by employees of the Sheriff's Office for cash or property that has been seized from an individual. These receipts are sometimes called cash seizure receipts, civil forfeiture receipts, or property for safekeeping or abandonment receipts. One copy of the receipt is given to the individual whose cash or property has been seized and another copy is kept by the Sheriff's Office.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

The Central Records Division does not generate or maintain any list as you are requesting. Since we are such a large organization all such cases are tracked by an item or docket number by the various bureaus and sections. If you have a particular case or cases we can research by these numbers.

I would like to amend my request to include the following records:
-Any and all copies of Form DAG-71(Application for Transfer of Federally Forfeited Property) submitted by the Jefferson Parish Sheriff's Office to any federal agency since 2009.
-Any and all communications since 2009 from the Jefferson Parish Sheriff's Office to the Jefferson Parish District Attorney notifying the DA's office of a property or cash seizure. Any Louisiana law enforcement agency "designated by the district attorney to make seizures for forfeiture shall notify the district attorney within ten days after such seizure," according to Louisiana law, RS 40:2606 subsection D.
-Any and all invoices or other comparable reports since 2009 that document the Jefferson Parish Sheriff's Office's contributions to the district attorney's Special Asset Forfeiture Fund and the proceeds the Sheriff's Office received from that fund as equitable distribution.

The Form DAG-71 is an agency specific form, the JPSO is not the custodian of those records. You may contact those specific Federal Agencies. Asset Forfeiture Forms are maintained by the Jefferson Parish District Attorney's Office; they are the custodian of those forms. They can be reached at (504) 368-1020.

Thank you for your advise regarding the DAG form. However, my request also included two other components that definitely fall under the purview of the Sherrif's Department:
