Public Records Request Log - Hawaii Environmental Health Administration

To Whom It May Concern:

Pursuant to Hawaii's Uniform Information Practices Act, I hereby request the following records:

Log of requests submitted under the Uniform Information Practices Act (UIPA) to the Department of Health's Environmental Health Administration that lists the date of request, the requester's name, the requester's organizational affiliation, a description of the request and whether or not the request was ultimately fulfilled. Ideally I would like this information for the period from 2013 to the present.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
