All materials and records describing and sufficient to show/disclose the following personnel information regarding GSP Corporal Joshua Bartlett :
- date of hire/promotion/firing
- changes in departmental assignments or assignment to/removal from departmental or governmental committees
- any and all commendations or awards
- any and all allegations of misconduct made and all records of any disciplinary proceedings and pursuant communications/outcomes from said proceedings
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The charges provided constitute a sum which is lower than the pro-rated salary for search, retrieval, review, redaction and production by each of the lowest paid full-time employees who, in the reasonable discretion of the Department, has the necessary skill and training to locate and provide responsive documents in each category listed. See O.C.G.A. § 50-18-71(b)(3).
• The charges provided constitute an automatic 15-minute reduction in each category of fees, which assures that such flat-fees are well-below the statutory guideline.
