Police Report - Serenity Gardens Funeral Home

To Whom It May Concern:

Pursuant to the Indiana Access to Public Records Act, I hereby request the following records:

Records related to the Gary Police Department's investigation of the former Serenity Gardens Funeral Home (934 East 21st Avenue), specifically the discovery of four sets of human remains and one set of cremains abandoned in the home in May 2009. Please include the responding officer's report, police narrative(s), any investigative reports produced, photographs, and interview transcripts or recordings. I have attached a completed City of Gary Law Department Request for Public Records form.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
