Autopsy & Investigative Report

To Whom It May Concern:

Pursuant to the Florida Sunshine Law, I hereby request the following records:

any and all public records related to Donnie Holland (date of death: 6/4/12), including, but not limited to: autopsy and investigation reports.

If your agency does not maintain these public records, please let me know who does and include the proper custodian's name and address.

I also request that, if appropriate, fees be waived as we believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage. All of the information received will also be shown on uncovered.com/cases.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

If you deny any or all of this request, please provide a written explanation for the denial, including a reference to the specific statutory exemption(s) upon which you rely, and notify me of the appeal procedures available to me under the law. Also, please provide all segregable portions of otherwise exempt material.
