Donald Holland Incident Report

To Whom It May Concern:

Pursuant to the Freedom of Information Act, I hereby request the following records:

Any and all records related to the 6/1/12 incident involving Donald Holland, including, but not limited to: time of report, narratives, incident reports, call logs (handwritten or electronic), notes and/or memos (handwritten or electronic), and any reports related to the the Raven .25-caliber handgun found in the SUV at the scene of the incident.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. If any or all of the request is denied, please provide a detailed explanation for the denial and the process for appealing the decision or remedying any errors in the request.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
