Pursuant to Utah’s Government Records Access and Management Act (“GRAMA”), I hereby request the following records:
the price of licenses for any electronic form software purchased by the City in the last five years
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

I would like to request the purchase order, contract, or proposal and any other documents related to a purchase from the vendor "Seamless Docs" or "Seamless Gov."

Kari,
I was recently made aware of a request for information that you filed on December 2, 2015. Unfortunately, the request was not sent to me so I was completely unaware that your request had been filed. Eagle Mountain City makes all of our public agreements available online on the City web page. Had the request been forwarded to me sooner, I would have directed you to the following site:

There is no associated PO since the contract was in place and the payment was made in accordance with the contract. A copy of the invoice is attached.
Please let me know if this satisfies your request as the original request was rather broad. If you are looking for email correspondence, there will be considerable cost and time associated with that since we contract for our IT services.
Please contact me with any future request for information as my office is responsible for this task. Thank you for your patience and please let me know if you have you need further clarification,
