Please provide a copy of all complaints (substantiated or unsubstantiated) or allegations of misconduct filed against the following police officers/detectives:

1. What is the total number of citizen complaints (allegations of misconduct, particularly, instability, hazing, harassment, direct intent to place vulnerable persons susceptible to serious health conditions in jeopardy, failure to intervene as required when witnessing a violent attack of innocent citizen, excessive force, threat of serious bodily injury, vandalism of personal property, theft of a motor vehicle, domestic violence, falsification of reports, malicious prosecution, falsification of search warrants claimed to have been legally approved, falsely reporting having presented legal search warrant and deceptively obtaining medical records illegally, Hippa violations regarding access to and the use of restricted and biased opinion to harass and intimidate persons legitimately seeking healthcare, criminal conduct in manufacturing alleged legally imposed restrictions creating trauma, stress, and anguish in falsifying what was an illegally imposed restriction to access to healthcare for a patient with a life threatening medical condition in illegally DEMANDING person may no longer access the healthcare they are personally contracted to receive, intentionally placing innocent persons in imminent risk of great personal harm and bodily injury as if person seeking competent healthcare and treatment were a terrorist even remotely requiring the ACTIVE SHOOTER/LOCKDOWN procedures allegedly enacted, assault with a deadly weapon, verbal threats of severe bodily injury based on intentional vandalism frothing police officer demanded was his right to inflict, and manufacturing fictitious 911 emergencies based on the delusional bias of one person bucking for police promotion while at the expense of an institution, coercion of witnesses, lies, and predatory conduct) made against these four officers annually for the last 10 years?

2. If your department logs complaints in a database, spreadsheet, or other log, a copy of those logs is fine instead. Please include the type of complaint (i.e., use of force, abuse language, unwarranted search) and the result of complaint (i.e., sustained, exonerated).

3. What is the total number of complaints police officers made against these four officers in department annually for the last 10 years? If your department logs complaints in a database, spreadsheet, or other log, a copy of those logs is fine instead.

Please include the type of complaint (i.e., use of force, abuse language, unwarranted search) and the result of complaint (i.e., sustained, exonerated).

4. What is the number of complaints made or found against all four officers involving dishonesty, illegal obtaining of evidence, lying under oath or in a report or statement made by these two acting under the scope of either a police officer, a detective or an Expert Witness, made in your department, for the past 10 years? Any documentation or reports that detail the information will suffice.

5. Please include documentation regarding how many of these complaints involved disciplinary action. Please include the number of times their conduct resulted in disciplinary action.

6. Which of the above data are published publicly? Please provide a link to any and all publicly available reports containing the data above.

7. Any complaints accusing any of these four officers of illegally obtaining evidence in order to find a just cause of action or probable cause, falsifying documents or statements, lying while acting as an Expert Witness, any allegations of dishonesty made against them, abusing their position of power, targeting specific groups of people based on their race or affiliations, any actions involving moral turpitude within the last 10 years, whether substantiated or unsubstantiated).

8. Any superior court lawsuits any of these four Dubois City Police Officers and/or citizen complaints, and/or peer complaints made by other police officers regarding these four Officer's conduct while acting under the Dubois City Police Department or other similar positions held since 2011.

9. Please provide ALL hospital, police, and other audio and video regarding the victimization of multiple hazing and assault incidents against the victim from the time period of January 1, 2015 to September 20, 2021 without delay.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
