Under the Texas Public Information Act, I hereby request the following records within 10 days per statute:

Any lists for deaths/examinations/autopsies performed by the Dallas County Medical Examiner and/or Southwest Institute of Forensic Sciences, Collin County Medical Examiner, Hunt County Medical Examiner, Rockwall County Medical Examiner, Kaufman County Medical Examiner, Ellis County Medical Examiner, Navarro County Medical Examiner, or for the government entities of Collin, Hunt, Rockwall, Kaufman, Ellis and Navarro counties in Texas since January of 2010.
I allow for the editing of information which is not able to be released under statute. No documents should be withheld, but only if necessary, information not able to be released under law may be edited.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
If there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
The requested records would be helpful if they include but not be limited to the following:
1. IFS Case Number
2. Decedent’s Name
3. Age
4. Date of Birth
5. Gender
6. Date of Death
7. Cause of Death
8. Manner of Death
9. County of Death
10. Place of Death
11. Address of Death
12. City of Death
13. Zip Code of Death

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

Hi Scott,
On December 31 , 2018, the Dallas County Medical Examiner's Office received your request seeking data for deceased individuals who passed away in several Texas counties during a specific time frame. See copy of the request attached to this email. As you may not know, the records you have requested are judicial records and are governed by Rule 12 of the Rules of Judicial Administration not the Texas Public Information act. Most of the Justices of the Peace have authorized this office to release all public data responsive to this request and will be made available within a reasonable amount of time. However, a portion of the data you seek was not authorized for release by the referring Justice of the Peace and/or may be excepted from disclosure under Rule 12.5 Rules of Judicial Administration.

Justice of the Peace Jackie Miller, Jr. - Ellis County Pct. 2 - does not authorize (cases will be removed from the listing)
Justice of the Peace Jack James - Rockwall County Pct. 1 - no response
Justice of the Peace Amy Tarno - Kaufman County Pct. 2 - no response
Justice of the Peace Mike Smith - Kaufman County Pct. 3 - no response
Justice of the Peace Johnny Adams - Kaufman County Pct. 4 - no response

There were two Justice of the Peace, one from Ellis County JP Chris Macon and one from Navarro County JP Darrell Waller that authorized most of the requested information on the cases sent to the Dallas County Medical Examiner, the two cases not authorized will be removed from the listing.

Since the Dallas County Medical Examiner is not the custodian of records for these judicial records, you will need to submit your request directly to the judicial officers named above as required by Rule 12 of the Rules of Judicial Administration. Additionally, a copy of your request has been forwarded to each Justice of the Peace named herein in compliance with Rule 12.

Hi Scott,
Please see attached spreadsheets by County. Please note, I have received authorization from JP Mike Smith in Kaufman County and the inquest data from Precinct 3 has now been included. As previously stated, the data from JP Jack James, JP Amy Tarno, JP Johnny Adams and JP Jackie Miller, Jr. have been excluded from the attached spreadsheets. Also, the two cases JP Macon and JP Waller requested be removed have been removed from their respective spreadsheets.

The data is from mid-April 2011 to current as this is the timeframe of our electronic system. Additionally, though the Medical Examiner ruled on the cause and manner of death for each of these cases, the referring Justices of the Peace actually certify the death. So, it is possible that the cause and manner could be different than what a particular Justice of the Peace certified on a death certificate.

The request was for specific counties and Dallas County was not listed so no spreadsheet was emailed. If you are needing that, please submit a new request to be processed. Also, as stated previously the data is from mid-April 2011 to current (date of request processing).
