In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Good afternoon,
In order to fulfill your autopsy report request for Jaime Tadeo Castillo (IFS-23-02346-ME), we will need payment mailed to us (exact cash, check, or money order only). Please see attached for the form to select which type of autopsy report you would like to order. Both autopsy reports have the toxicology report included in it.
You can also request the investigation narrative report. This report includes what the death investigator observed at the scene of the decedent's death and information reported about their death. The fee for the investigation narrative report is 0.10 cents per page. His report has 3 pages, (0.10 cents x 3 pages = 0.30 cents due). Please see the attached form if you would like to request this report. Please mail your payment with the form attached.
