Pursuant to the Texas Public Information Act, I hereby request the following records:
I am requesting a copy of the completed autopsy report for Cleveland Kent Berry, DOB 9/12/1957, DOD 9/4/2018 or 9/5/2018.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Hi Mr. Mixon,
We have received your request for the autopsy report of Cleveland Kent Berry and at this time it is incomplete. It takes approx. 90 for an autopsy report to finalize. There is a fee of $5 for a non-certified copy of an autopsy report and $15 for a certified copy of an autopsy report. You may send in payment and your request will be filed when the report is finalized and payment is received then processed.
