Pursuant to the Connecticut Freedom of Information Act, I hereby request the following records:

(1) This request is for all records including but not limited to, any and all reports, supplements, narratives, statements, photographs, transcripts, evidence logs/inventories, field notes, texts, emails, electronic application communications, audio recordings, video recordings, interview notes, and/or telephone logs/voicemails, regarding CFS# 1900499754. The specification of certain records does not limit the request in any manner to just those records.

(2) This request is for all records including but not limited to, any and all reports, supplements, narratives, statements, photographs, transcripts, evidence logs/inventories, field notes, texts, emails, electronic application communications, audio recordings, video recordings, interview notes, and/or telephone logs/voicemails, regarding any and all complaints made by Melissa Brezniak about Dominic Goguen to any person employed by the State of Connecticut, including but not limited DEESPP and the Division of Criminal Justice. The specification of certain records does not limit the request in any manner to just those records.

(3) This request is for all records including but not limited to, any and all reports, supplements, narratives, statements, photographs, transcripts, evidence logs/inventories, field notes, texts, emails, electronic application communications, audio recordings, video recordings, interview notes, and/or telephone logs/voicemails, regarding complaints made by Melissa Brezniak about Dominic Goguen to any person employed by the State of Connecticut, including but not limited DEESPP and the Division of Criminal Justice on 10/17/19, 2/22/21, and/or 4/30/21. The specification of certain records does not limit the request in any manner to just those records.

(4) This request is for all records including but not limited to, any and all reports, supplements, narratives, statements, photographs, transcripts, evidence logs/inventories, field notes, texts, emails, electronic application communications, audio recordings, video recordings, interview notes, and/or telephone logs/voicemails, regarding Dominic Goguen's and/or Attorney Rachel M. Baird's, requests for records regarding CFS # 1900499754 in December 2019 and directly to DESPP legal affairs in April 2021 or at any other time such request was made. The specification of certain records does not limit the request in any manner to just those records.

(5) This request is for all records including but not limited to, any and all reports, supplements, narratives, statements, photographs, transcripts, evidence logs/inventories, field notes, texts, emails, electronic application communications, audio recordings, video recordings, interview notes, and/or telephone logs/voicemails, regarding the Administrative Inquiry addressed by Lt. Daniel Loughlin #51 by letter dated April 22, 2020. The specification of certain records does not limit the request in any manner to just those records.

THESE REQUESTS WILL AUTOMATICALLY RENEW EVERY TWENTY-NINE (29) DAYS COMMENCING ON THE DATE OF THIS REQUEST.

I also request that, if possible and pursuant to Conn. Gen. Stat. ยง 1-212(d), fees be waived as I believe this request is in the public interest and not made for commercial gain. The requested documents will be processed by a representative of the news media/press and made available to the public at MuckRock.com.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hello,

This is a reminder of the requirements for the submission of evidence and witness list. If you have not already submitted them, please be sure to do so at least three (3) business days prior to the scheduled contested case hearing on 1/13/2022 at 2:00 PM.

If you are sending them by email, please email them to FOI@CT.GOV.

For your convenience I have included the requirements that were included in the Order to Show Cause.

REQUIREMENTS FOR SUBMISSION OF EVIDENCE AND WITNESS LIST:

* At least three (3) business days prior to the scheduled contested case hearing, each party shall disclose to all other parties all proposed exhibits or other written materials that the party intends to use as evidence at the hearing and shall simultaneously provide a copy of such exhibits to the hearing officer. Each proposed exhibit or other written document must be individually paginated on the bottom right corner. Please do not premark the proposed exhibits.

* At least three (3) business days prior to the scheduled contested case hearing, each party shall disclose to all other parties a list of the individuals that the party will call to testify at the contested case hearing and shall simultaneously provide a copy of such list to the hearing officer.

* Any party intending to call witnesses for testimony must ensure that such witnesses have received the videoconference remote proceedings link and a copy of all exhibits or other written materials that have been exchanged between the parties.

* Disclosure of proposed exhibits or other written materials may be done by fax, email, or regular mail. Please note that any proposed exhibits or other written materials in excess of fifty (50) pages must be submitted to the hearing officer by regular mail or courier.

* Failure to provide proposed exhibits or other written materials to the other parties and the hearing officer at least three (3) business days prior to the contested case hearing may result in the exhibits not being admitted for consideration at the hearing.
