Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
All completed Gang Audit Questionnaire documents (see: directives.chicagopolice.org/forms/CPD-12.140.pdf) dating from January 1st, 2018 to May 31st, 2018.
In order to easier find responsive records to this request, see the brief description of data logistics/retention on the top of the the CPD-12-140 form:
"Use this form to add, delete or change information and circle the appropriate action on the right. If a Gang Faction is being added, include all the applicable information as shown below. Make extra copies of the questionnaire if necessary and indicate page number as appropriate on bottom. Email the completed questionnaires to gangaudit@chicagopolice.org."
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please be advised that after contacting both the Bureau of Organized Crime and CPIC, both indicated that they had no responsive records for this request. I was told that the Bureau of Patrol may have this information. I have recently contacted this section, and I hope to receive a reply this week. Hopefully, this section will have responsive records, but if not, I still have to wait to see if they would be able to let me know who would have it since the other sections do not.
