Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
Documents sufficient to show all payouts the Chicago Police Department made for damage its department or staff inflicted on private citizens' property, from Jan. 1, 2010 through Sept. 21, 2016; documents sufficient to show the amounts of said payouts; documents sufficient to describe the property the payout was for; documents sufficient to show the date of the incident wherein said property was damaged; documents sufficient to show the date of the payout. It is possible that the full request could be fulfilled with a single database or spreadsheet, but the request is for documents sufficient to show all the above criteria. If separate databases or spreadsheets are used in the response, please do not redact any unique identifiers (for example, unique numbers assigned to the incidents or the payouts) that CPD places on the incidents or the payouts.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I request also documents sufficient to show the location of each incident wherein CPD damaged a citizen's property. This could be in the same spreadsheet or database as above. I also request documents sufficient to show applications for property reimbursement that were not approved by CPD, in the above date range.
