Pursuant to Tennessee's Public Records Act, I hereby request the following records:

I would like a copy of the arrest video for DUI/Implied Consent from November 2009 for Janet Kaye Wyatt, birthdate 2/20/1961.

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

I received the request for the video, and sent it via USPS Priority 2-Day mail. According to the USPS tracking information, it was delivered to the provided address (MuckRock News Dept MR 14104 PO Box 55819 Boston MA 02205) on November 12th, 2014. This video is too large to email and can only be provided on a CD.

If you have an alternate means to accept the video, please let me know, and will make another attempt.

I have tried repeatedly to open the download and open the file of my video from the actual CD that MuckRock sent me. Now I have viruses and my security scan does not recommend the download. PLEASE send me a video or DVD I can play of my arrest.
