Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
- The city's budget for fiscal years 2011, 2012, 2013, 2014 and 2015 for take-home vehicles for city employees
- A list of city employees who received a take-home city vehicle in those fiscal years, the dates they received it and relenquished it (if they have yet relenquished the car). Also include the positions and job descriptions of the people on this list.
- The city's contract with the company that maintains the city's car fleet.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Mr. Smith, Attached you will find your FOIA request, our #16-039. I apologize, once again my office did not receive this back in December of 2015. The e-mail address that it was sent from was identified as a spam e-mail and ended up in my spam box of 2-2-16, (see attached screen shot) as it may have back in December, as well and had gone undetected. Below is the response from the City of Berwyn.

In addition to the individuals and their Job descriptions contained in the attached PDF, Berwyn City Administrator Brian Pabst (see attached job description) and Public Works Director Robert Schiller also have take home vehicles.
