Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
- Total amount paid out by the City to the companies 1. Suburban Windows and Siding and 2. JNC Consulting, in the calendar years 2011, 2012, 2013, and 2015 up to Dec. 9, 2015.
- Any documentation (such as work history, certifications or qualifications, or bid proposals) submitted by these companies to the City of Berwyn before they were hired to do work for the city.
- How many jobs these companies each received in each of these calendar years.
- A copy of all building department permits that had their fees waived in the calendar years 2012, 2013, 2014, and 2015.
- A copy of all FOIA requests submitted by the Better Government Association to the City of Berwyn from Jan. 1, 2014 through Dec. 9, 2015.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Mr. Smith, Attached you will find your FOIA request, our #16-036. I apologize, once again my office did not receive this back in December of 2015. The e-mail address that it was sent from was identified as a spam e-mail and ended up in my spam box of 2-2-16, (see attached screen shot) as it may have back in December, as well and had gone undetected. Below is the response from the City of Berwyn.

- How many jobs these companies each received in each of these calendar years. The City of Berwyn does not maintain a file on the number of jobs a company receives in any given year.
- A copy of all building department permits that had their fees waived in the calendar years 2012, 2013, 2014, and 2015.Note: The City of Berwyn does not maintain files on building department permits that are waived in any given year.
- A copy of all FOIA requests submitted by the Better Government Association to the City of Berwyn from Jan. 1, 2014 through Dec. 9, 2015. (see attached)
