Records specific to an employee

To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

Any and all records, data or documents associated with the City of Berwyn employee Ruth Volbre, to include but not be limited to the following personnel records, yearly performance reviews, professional certifications, awards for accomplishments, disciplinary paperwork associated with the employee, and documents sufficient to show length of employment/tenure in this position and all previous positions with the city, job descriptions of all positions with the city, and communications between Ruth Volbre and anyone within the Office of the Mayor of Berwyn. Please respond directly to the email address the letter was sent from.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
