Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
- Any contracts, memoranda of understanding, of other written agreements between the City of Berwyn and R.E. Walsh and Associates (Hereafter, R.E. Walsh)
- All billing statements from Jan. 1, 2010 to December 9, 2015 and the purpose of the payment to R.E. Walsh.
- Any and all subpoenas issued to the City of Berwyn and tendered to or from R.E. Walsh and City of Berwyn.
- All email communication between representatives of the City of Berwyn and R.E. Walsh, from Jan. 1, 2010 and December 9, 2015. This includes from personal email addresses of employees of the City of Berwyn, if any city business was discussed via personal email accounts.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Mr. Smith, Attached you will find your FOIA request, our #16-038. I apologize, once again my office did not receive this back in December of 2015. The e-mail address that it was sent from was identified as a spam e-mail and ended up in my spam box of 2-2-16, (see attached screen shot) as it may have back in December, as well and had gone undetected. Below is the response from the City of Berwyn.
