Any reports of Medicaid fraud or abuse made about the following 15 healthcare facilities, or employees of those facilities, from January 1, 2015 to the present.

- Ascent: North Little Rock Day Treatment & Outpatient Clinic
- Ascent: Paragould Day Treatment & Outpatient Clinic
- Ascent: Trumann Day Treatment & Outpatient Clinic
- Ascent: West Memphis Day Treatment & Outpatient Clinic
- Millcreek Behavioral Health
- Piney Ridge Center
- Riverview Behavioral Health
- Valley Behavioral Health System
- Vantage Point Behavioral Health System
Any internal DHS office email correspondence regarding possible Medicaid fraud or abuse at those facilities.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

Mr. Millar,

We just received a “follow up” request (the same request) from Muckrock. I’m advising you of this because once again, the request is dated several days ago (9/6/18) and appears to have been sent via fax, but it has only just now made its way to the FOIA office. Would you please advise them the most efficient way to send a FOIA to DHS is through the email address on our website, which is:
