Food Inspection Reports -- 2018 to 2023

To Whom It May Concern:

Pursuant to the Arkansas Freedom of Information Act, I hereby request the following records:

A listing of all food inspections conducted by the Arkansas Department of Health from 1/1/2018 to the present. In particular, I request a spreadsheet, database, or delimited text file(s) — not paper records or a PDF created from data — with the following columns/fields/variables:
- establishment name
- license/permit number
- establishment address
- establishment city/state
- establishment zip code
- establishment type
- permit holder
- inspection reason
- risk category
- The establishment's rating (in compliance, out of compliance, not observed, or not applicable) for each of the 29 risk factors and 28 good retail practices listed on the Food Establishment Inspection Report form, including whether any issue was corrected on site during inspection and/or was a repeat violation.
- If applicable, the item number, code violated, priority level, comment, and correct by date of any observations and corrective actions.
- all temperature observations
- inspection visit date
- person in charge at establishment during the inspection
- full legal name of the inspector
- time in and out of inspection

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
