Pursuant to the Nevada Public Records Act, I hereby request the following records:
1. Records, reports, and communications with Eddy House concerning requests for any fee deferrals from February 2019 through September 2020.
2. Records or reports from inspections of facilities maintained or owned by Eddy House from February 2019 through September 2020.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

We have received your request for information regarding Eddy House. Unfortunately, we were unable to locate the request as being received in our office last October. However, we have no records for Eddy House for the dates requested as they were not permitted and under our jurisdiction at that time. Please let us know if you have any further questions.
