Pursuant to the Government Records Access and Management Act, I hereby request the following records:
All contracts, licenses, MOUs, MOAs, data agreements, letters of intent, State of Work documents, Scope of Work documents, and other agreement documents with Pat V Mack Inc. that were active any time between 1/1/2020 and 1/1/2024.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

The State of Utah Division of Technology Services (DTS) has received your
request for the following records:
*All contracts, licenses, MOUs, MOAs, data agreements, letters of intent,
State of Work documents, Scope of Work documents, and other agreement
documents with Pat V Mack Inc. that were active any time between 1/1/2020
and 1/1/2024.*
Your request has been approved. Attached are the purchase orders,
statements of work, and invoices. DTS is not the owner of the contract
records. If those records exist, the State of Utah Division of Purchasing
is the owner of those records.
In the future, we recommend submitting records requests through the State
of Utah Open Records portal at openrecords.utah.gov.
