Related to the Independent Assessments and Transformation Support solicitation (The solicitation was released on 11/8/16, the due date was 12/5/16, and the award time frame occurred during 1/2017).
1. A copy of the contract and any amendments
2. Copies of any invoices since contract award
3. Copies of the compliance reports listed below:
Monthly Program Performance Report
Annual Program Performance Reports
Transformation Support and Training - Monthly Progress Report
Transformation Support and Training - Quarterly Practice Participation Report
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

We have received this request for the first time today. I'm not sure where it went prior to today, though the Office of State Procurement is not a part of the Department of Human Services. Regardless, the Arkansas Freedom of Information Act is available only to citizens of the State of Arkansas, so we must deny your request.
