Pursuant to the Freedom of Information Act, I hereby request the following records:
I am seeking to obtain the OPAIS database inclusive of the covered entity's email addresses for the Authorizing Official, the Primary contact, and the contract pharmacy's responsible party's email address. the current website located at https://340bopais.hrsa.gov/CoveredEntitySearch/000007820 does not contain the email address of the parties.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 20 business days, as the statute requires.

In the prior request, Human Resources and Services Administration (HRSA), Office of Pharmacy Affairs (OPA), searched and located one Microsoft Excel workbook (16,299 KB), which was released in its entirety. I can share the previously released record (Excel workbook) with you through the NIH Secure Email and File Transfer Service (SEFT). Please let me know if you would like me to send you a copy.
