Pursuant to the Freedom of Information Act, I hereby request the following records:

The Official Military Personnel File (OMPF) for PER ERIK TORNBLOM including Form DD-214, all Fitness Report and Counseling forms NAVPERS Form 1610/2 - 1610/1, Enlisted performance evaluation reports NAVPERS Form 1616-1, the Background Summary (NAVPERS Form 1640/32), Cumulative Record of Training (NAVPERS Form 5300/16), history of assignments NAVPERS 1070/605, Navy Training History NAVPERS form 601-4, all military decoration and citations, military training courses.
Specifically request copy of history of assignments NAVPERS form 1070/605, copy of Bronze Star Medal citation and Navy commendation medal citation. Mr Tornblom served in South Vietnam during the Vietnam war. Please redact any sensitive information protected in accordance with the Privacy Act of 1974.
