Pursuant to FOIA Federal Statute: Title 5 of the United States Code Section 552

I am requesting that HRSA provide me with all DEA numbers and related Contract Pharmacy IDs (matching the field "Pharmacy ID" per the Contract Pharmacy Daily Report found at https://opanet.hrsa.gov/340B/Views/Reports/DownloadReport.aspx?reportName=CP\OPACP_DAILY) for 340B contract pharmacies that exist in the HRSA 340B database.

This request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
