Dog licenses in South Bend, IN

I am the Manager of Animal Care and Control in South Bend, IN. In order to fill this request, our city legal department does require you to fill out the city form found at http://southbendin.gov/government/content/public-records-request.

If you could please clarify what you are looking for in your request of the current Dog Registration of Dog License Database, that would be most helpful.
