Pursuant to the California Public Records Act, I hereby request the following records:

COI for Sutton Enterprises Vallecito, CA Sutton enterprises were hired by Paradise Irrigation District to install new water piping . Damage occurred in
the front easement of property. While digging to replace piping, Sutton damaged a .625 Feeder cable that ran
through their work area. Sutton enterprises had a valid USA # X 019502131 and marks were correctly
painted on ground by Utiliquest . USA marks were still visible day of damage and could be seen around
damage area. More precaution should have been taken when working around Comcast Marks .

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Ms. Kramer, the Town of Paradise has no contractual relationship with Sutton Enterprises, and therefore, does not have a current copy of their certificate of insurance. This information should be easily obtained through their contractual relationship with Paradise Irrigation District relating to water service line replacement efforts. Please confirm receipt. Dina
