Oregon COVID-19 Workplace Complaints

To Whom It May Concern:

Pursuant to the Oregon Public Records Law, I hereby request the following records:

A copy of complaints regarding unsafe COVID-19-related working conditions submitted between February 1, 2020, through the date this request is processed.

A similar database was released to Willamette Week, according to their recent reporting:

I request a copy of the same database released as part of that request, along with any additional entries submitted since that request was fulfilled.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
