Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

All use of force reports from January 1, 2017 to December 31, 2017 including the following information, where available:

The gender, race, age and name of each individual who was shot, injured, or killed.
The date, time, and location of the incident.

Whether the civilian was armed, and, if so, the type of weapon.
The type of force used against the officer, the civilian, or both, including the types of weapons used.
The number of officers involved in the incident.
The number of civilians involved in the incident.
A brief description regarding the circumstances surrounding the incident, which may include the nature of injuries to officers and civilians and perceptions on behavior or mental disorders

For location of the incident, please provide a latitude and longitude, if recorded. If not, please provide the street address including zip code.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I have included the data that I was able to extract from our RMS and internal investigation systems. I could not include race with the subject’s name, because under FOIA that is considered a personal identifier.
