From: Sarah Emily Baum

To Whom It May Concern:

Pursuant to the New York Freedom of Information Law, I hereby request the following records:

Data surrounding COVID-19 rapid test kit distribution.

As per schools.nyc.gov, the New York City schools are "distributing free, take-home COVID-19 rapid test kits to any students or staff who exhibit COVID-like symptoms or have been in a classroom where a positive case has been identified." I would like to know how many of these rapid tests total have been distributed from the beginning of this initiative until now. In addition, if possible, I would like to know demographic data broken down -- which schools or zip codes received how many tests on what dates, as well if there is any info on if tests went to students or staff.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Sincerely,

Sarah Emily Baum
