pet name
pet age
owner name (if possible)
owner address (if possible)
owner zip code
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The NYC Department of Health and Mental Hygiene (DOHMH) acknowledges receipt of your Freedom of Information Law request. Since we didn't received your request on February 4, 2020, it has been assigned the above-noted control number and has been processing now as new request.
Due to the current COVID-19 emergency, DOHMH is focused on responding to the city's public health needs. Like other city agencies, all staff not essential to this response are working remotely.
