Pursuant to the New York Freedom of Information Law, I hereby request the following records:

Records showing, for each inactive and active uniformed member of service of the NYPD, the following information:
* MOS tax ID
* MOS sex
* MOS race/ethinicity
* MOS age (or year of birth)
* Date MOS was appointed to NYPD
* History of ranks held by MOS, with dates of changes in rank and shield number associated with each rank held by MOS
* History of command assignment of MOS, with date MOS was assigned to each new command
* MOS highest level of educational achievement
* Each allegation of misconduct lodged with the CCRB against the MOS, with the following information:
-- CCRB case number
-- IAB Log number if available
-- agency that allegation was investigated by (ie please indicate referrals to IAB, Chief of Department, or other agencies, or if CCRB kept the allegation)
-- whether the allegation was fully investigated
-- race, sex, and age of the allegation victim
-- if possible, a unique identifier for alleged victims so that victims filing more than one complaint can be connected across multiple cases
-- precinct of occurrence
-- address of occurrence (can be anonymized to the nearest 100 street address or to street segment/intersection)
-- reason for initial contact
-- criminal justice outcome of incident (e.g. arrest, summons, neither)
-- whether BWC footage was available
-- whether video was available from another source
-- whether victim was injured
-- whether officer was injured
-- FADO category and specification (e.g. search, physical force, rude gesture, etc) of misconduct alleged
-- date of occurrence, date of filing of complaint, date of filing of recommendations by CCRB staff, date of disposition by CCRB Board for each allegation
-- outcome recommended to the CCRB Board by staff
-- CCRB Board disposition
-- For substantiated allegations, whether DAO or APU prosecuted and the disposition (ie dropped by prosecutor, pleaded guilty, trial)
-- For cases tried, whether tried at OATH or DCT and the recommendation of the trial hearing officer/commissioner/ALJ
-- Final disposition of allegation by the Police Commissioner
-- for Command A or Command B disciplinary outcomes (pleas, summarily imposed, or after trial), please give the actual penalty imposed at the command

Please provide the information electronically in a spreadsheet format (Excel, .csv) if possible, with all text machine-readable.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
