To Whom It May Concern:

Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:

A listing of every officer and/or employee of the agency, including name, office address, title and salary.

Per NEW YORK PUBLIC OFFICERS LAW, ARTICLE 6, Section 87(3)(b):

"Each agency shall maintain:
[....]
(b) a record setting forth the name, public office address, title and salary of every officer or employee of the agency."

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Sincerely,

Benjamin Lesser

Hello,

I am attempting to confirm the contact information for submitting FOIL requests to FDNY.

As part of a project in collaboration with Mr. Benjamin Lesser of Columbia Journalism School, I have sent four FOIL requests to the following address without reply:

Record Access Officer
FDNY Bureau of Legal Affairs/CDLS Unit
9 MetroTech Center, 4th Floor
Brooklyn, New York 11201-3857

Please confirm that this is the correct address, and whether these four requests have been received. If this is not the correct address, or if there is a better means of submitting FOIL requests to FDNY, please advise.

Thank you,
Shawn Musgrave

From: Maura.Kugelman@fdny.nyc.gov

The address is accurate. Thank you.

From: MuckRock

Hello,

Thank you for confirming that the address is correct.

Can you confirm whether Mr. Lesser's FOIL requests mailed to that address have been received? The requests are for the following documents:

1) FOIL rules and regulations
2) FOIL request log
3) Subject matter listing
4) Employee listing

The requests were mailed to the address below on January 14, 2015, and then a followup was mailed on January 30. To date, there has been no acknowledgement or other communication received for any of the four requests.

Thank you again for your assistance.

From: Maura.Kugelman@fdny.nyc.gov

I can only confirm that two requests have been received and entered into our database as of 02/23/2015:

1. Subject Matter Listing

2. FOIL Request Log

"Each agency shall maintain:
[....]
(b) a record setting forth the name, public office address, title and salary of every officer or employee of the agency."
