Pursuant to the New York Freedom of Information Law, I hereby request the following records:

On December 20, 2011, the New York State Department of Corrections directed the NYC Department of Health and Mental Hygiene and the director of the Health and Hospital Corporation to take the following actions following the Feb 3, 2011 death of Rikers Island inmate Aris Hiraldo.

-to conduct an inquiry into Corizon Health's "fitness to conduction prescription medication administration and directly observed therapy" at Rikers Island, specially inquiring into Corizon's medication delivery/administration credentialing and qualification system for pharmacy technicians

Please provide copies of any documents created as part of this inquiry, including memos, investigative reports, or recommendations

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
